survey for elemntor website (wordpress)

hey there
how can i create a small survey of 3-4 qustions on my website? i want it show *in the same page* when a visitor clicks on the button (this is just a 3 -4 qustions which every qustion got 4 answers for exemple) and after the visitor answer it all, a popup opens and it some text will show there and an option to add a button there

i want my visitor more engaged
